The key to our success is our system of comprehensive clinical case management, allowing us to coordinate and monitor all homecare services for our patients. Serving as a single source solution, we schedule caregivers, order equipment and pharmaceuticals, organize community resources, and coordinate insurance benefits.
We are currently seeking qualified individuals for cases in the Greater Springfield, Wales, Greenfield, Pittsfield, Ware, Agawam, Chicopee, Holyoke, and Northampton areas.

Requirements
-Home Health Aide Certificate
-Healthcare Provider CPR card, from the American Red Cross, or American Heart and Lung Assco.
-Strong communication skills
-Reliable Transportation
-Ability to work independently
